Key Responsibilities
Evaluate member needs and recommend care plans
Develop ongoing long-term care plans
Coordinate between members, families, and care providers
Provide resource support for members
Monitor and document care plans and member outcomes
Educate members and families on healthcare options
Provide feedback to improve care quality
Required Qualifications
Bachelor's degree
2 – 4 years of related experience
Driver's license
