Job Description

We are currently seeking a skilled and compassionate Primary Care Nurse Practitioner to join our team in Defiance, Ohio. This is a full-time outpatient role focused on providing primary care services to adult and geriatric patients. The ideal candidate will bring strong clinical expertise in adult-gerontology and a passion for delivering patient-centered care.

Key Responsibilities:

• Provide primary and preventative care for long term care patients residing in skilled nursing facilities.
• Manage Residents with acute change in condition.
• Advanced care planning and chronic care management.
• Prescribe appropriate medications, including controlled substances in accordance with OH Law.
• Order, review and/or follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular basis and as needed to discuss changes in condition.and/or the care plan.
• Collaborate with a primary care physician.

Required Qualifications:

• Master’s Degree in Nursing with a specialization in Adult-Gerontology Nurse Practitioner (AGNP) – Required
• Current Nurse Practitioner License in the State of Ohio – Required
• Board Certification as an Adult-Gerontology Nurse Practitioner (AGNP) – Required
• DEA License Required
• Minimum 2 years of experience in primary care, internal medicine, or geriatric care preferred
• Strong clinical judgment, communication, and interpersonal skills
• Proficient with electronic medical records (EMR) systems
