Home Health Occupational Therapist Full Time 30 Hours

Key Responsibilities

• Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and current level of function
• Administer, evaluate, and interpret tests and measurements to determine functional goals and outcomes
• Provide a written evaluation and plan of care defining occupational therapy needs, goals, progress, and expected outcomes
• Deliver occupational therapy services to comply with prescribed physician orders
• Maintain plan of care by performing timely documentation of client performance and modifying goals and treatment as needed
• Responsible for discharge planning and educating the patient and caretakers on home exercise plans
• Participate in coordination of services with healthcare team members
• Document according to policy and procedure with minimal supervision
• Maintain effective communication between staff, healthcare team members, and family
• Supervise other personnel (OTA, HHA) as applicable
• Complete OASIS assessments where allowed by state professional practice and regulation
