Patient Ratios Days: 20
Orientation Hours: 40
Patient Age Groups: Adults, Geriatrics
Daily Census: case load up to 25 patients
Interdisciplinary Support: Physical Therapy, Respiratory Services, Interpretation Services, Phlebotomy, Radiology, Social Services, Rapid Response Teams, Pharmacy, Transportation, Unit Secretary
Patient Diagnoses: Should have extra checklist w/ NURSYS Acute Hospital Case Manager experience is a HARDSTOPINTERQUAL is a HARDSTOP
Special Procedures/Unit Details: Ability to organize and participate in family meetings.
Participate in the development of the treatment plan. Experience with HMOs, IPAs, etc.
Competent with arranging DME, SNF and ambulance transport.
Must be able to perform this task independently.
Special Equipment: Computerized documentation, required. Weekend rotation Case Load up to 25 patients.
