Care for seriously ill patients to alleviate burdensome symptoms
Align medical treatments with patients' goals and values
Collaborate with a team of clinicians to deliver quality care
Provide ongoing palliative care and hospice service and education
Required Qualifications
Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing
2 years experience in chronic disease management/long-term care
Reliable transportation
Ability to lift at least 80-100 lbs
