Key Responsibilities
Conduct in-home assessments including evaluation of medical history and vital signs
Perform evidence-based practice screenings
Identify diagnoses for care management
Communicate findings to inform member’s PCP
Identify urgent situations and intervene
Educate members on disease processes and medication compliance
Required Qualifications
Active NP license or ability to obtain by start date
Active ANCC or AANP national certification
Ability to complete physical requirements of the job
Driver's license and reliable transportation
Preferred Qualifications
1+ years of clinical experience
Experience in gerontology, cardiology, internal medicine, or endocrinology
Home Health care or home visit experience
Ability to travel up to 5% within licensed states
