Case Manager RN needed to coordinate utilization management, discharge planning, resource management, and care facilitation across the acute care continuum. The RN will manage high-risk patients, focusing on reducing unnecessary admissions/readmissions, ensuring appropriate post-acute care referrals, and minimizing payment denials. Role includes inpatient and ED coverage.
