Job Description
Join The Jewish Board as a Case Associate and make a significant impact by supporting clients in accessing vital resources and services. You will work closely with clients and communities to develop personalized action plans, ensuring their needs are met while providing essential case management and counseling.
Key Responsibilities
Function as a case planner to supply appropriate interventions for client care
Develop action plans to utilize existing and new resources for clients
Perform routine community contacts and home visits
Evaluate living conditions of clients
Provide structure and guidance in independent living skills
Facilitate client referrals and access to treatment or service providers
Perform individual/group counseling
Conduct community outreach and establish linkages with organizations
Maintain necessary documentation and progress notes
Required Qualifications
BA Degree
Experience in a community or social service group or 5 years as a Case Assistant
Preferred Qualifications
Bilingual Haitian-Creole
