Key Responsibilities
Carries a caseload and practices person-centered service planning
Evaluates independent living skills and supports residents in meeting goals
Advocates on behalf of residents with service providers
Completes documentation of resident interactions
Participates in intake, assessments, and follow-up conferences
Required Qualifications
Bachelor of Arts (BA) or Bachelor of Science (BS) degree
At least 1-year experience in Human Services preferred
In-depth understanding of mental illness, substance use disorders, or trauma
Experience working with individuals living with serious mental illness
Preferred Qualifications
Familiarity with Microsoft Office suite
Valid driver's license and willingness to drive agency van
