Key Responsibilities
Assess, plan, implement, and coordinate care management activities
Develop personalized care plans/service plans
Educate members and families on services and benefits
Coordinate care between members, families, and providers
Monitor progress towards care plan goals
Provide psychosocial support and resource access
Collaborate with healthcare partners
Document member information and care management activities
Perform on-site visits and provide education to care managers
Serve as a Subject Matter Expert (SME) for the care navigation team
Required Qualifications
Bachelor’s degree
4 - 6 years of related experience
Licensed Practical Nurse (LPN) with a minimum of four years of pediatric experience or a Master’s degree in social work with a minimum of one year of related professional pediatric care experience
Preferred Qualifications
Current state’s clinical license
Experience in care coordination
Strong communication skills
