Join Monte Nido & Affiliates as a Hybrid Social Worker, where you'll play a vital role in care coordination and case management for clients in our intensive treatment programs. Collaborate with a multidisciplinary team to ensure clients and families have seamless access to essential community resources.
Key Responsibilities
Care Coordination & Case Management: Partner with the clinical team to offer case management and care coordination for clients and families.
Community & Family Support: Assist in connecting clients and families with relevant community-based programs.
Referral Management: Send referrals to community programs and resources.
Transitions Between Care Levels: Help facilitate smooth transitions between various levels of care.
Prepare Clients & Families for Transition: Provide necessary information and emotional support.
Ongoing Resource Connection: Ensure clients continue to receive care after transitioning.
Required Qualifications
Master’s degree in Social Work or related field
Experience with CareOregon/Medicaid
Proven experience in case management or care coordination, preferably in behavioral health or residential treatment settings
Strong communication skills
Ability to work collaboratively with a multidisciplinary team
Ability to build relationships with clients, families, and community providers
Understanding of available community resources, government programs, and insurance processes
Preferred Qualifications
LCSW preferred
