Nationwide Therapy Group is seeking a travel nurse RN Case Management for a travel nursing job in Santa Monica, California.

- Conduct biopsychosocial assessments to evaluate mental, emotional, and environmental factors impacting patient care.
- Develop individualized Plans of Care (POC) in collaboration with the interdisciplinary team.
- Participate in treatment team meetings and care conferences.
- Identify and address high-risk patients, including those at risk of homelessness , mental health crisis , or drug/alcohol abuse .
- Handle 51/50 psychiatric holds , suicidal ideation , and mental health crisis interventions .
- Provide crisis counseling , family therapy , and advance directive planning .
- Make appropriate protective service referrals (APS, CPS).
- Coordinate transitional care services including DME, home care, and facility placement (rehab, SNF, LTC).
- Secure prior authorizations and assist with insurance navigation (HMOs, IPAs).
- Refer patients to external community resources for ongoing care.
- Support patients in establishing guardianships , powers of attorney , and advance directives (POLST, living wills).
- Accurately document:
- Response to treatment
- Plan of care implementation
- Interventions and referrals
- Understanding of Medicare/Medicaid coverage criteria
Licensure: Active LCSW or MSW with CA license Minimum 1 year inpatient experience Prior experience in crisis intervention, ED, and discharge planning Charting: Proficiency with Epic
