Key Responsibilities
Conduct Adult/Geriatric assessments including medical history and diagnosis
Work collaboratively with physicians and case managers
Provide follow-up and patient education
Conduct services in various venues including home visits
Required Qualifications
Master’s Degree or commensurate experience
Current RN and NP licensure in state of practice
Board certified by AANP, ANCC or AACCN
Current BLS, ACLS or CPR certification
1 year experience as a Nurse Practitioner preferred
Strong computer skills
Excellent verbal and written communication skills
Comfort and flexibility with frequent change
Preferred Qualifications
Experience with employee health/medical record software
