We’re seeking a part-time *Transfer Access Navigator *to join our EpiCenter department for the night shift at Rhode Island Hospital in Providence, RI. In this position you will assist with all aspects of patient care, including diagnosis, treatment and consultations. We’ll rely on you to manage patient flow, access capacity and transport teams, ensuring the right patient is in the right place at the right time.
*Responsibilities:*
· Facilitate clinical and administrative transfers and collaborate with nursing departments and physicians to ensure smooth patient movement within the health system
· Perform physical exams and document medical histories, progress notes and discharge summaries, and write operative procedure notes for all critical and non-critical patient transports
· Coordinate transfer requests, provide information regarding transport of patients to and from Brown University Health institutions, and provide monitored communication with transport team for patient consultation, referral and estimated times of arrival and departures with receiving site
· Coordinate demographic and insurance information with Admitting Departments and case manager across BUH system on repatriation processes and agreements
· Work with capacity and flow navigators to understand bed availability across the system and triage any concerns or issues that would impact a site’s ability to accept a patient
· Participate in QA in specific/daily meetings using established quality indicators for department, and engage in community outreach and in-house educational efforts, such as nursing grand rounds
*Qualifications:*
· BSN preferred; must complete Transfer & Access center credentials within 3 months
· Current RN license in RI (or a state that participates in the Nurse Licensure Compact) * 3+ years of current experience as a professional nurse providing care within an intensive care unit or emergency department * Strong computer skills with knowledge of EHR (Epic) computer programs and Microsoft Office for retrieval of demographic information performance data and statistics * Highly effective verbal and written communication skills to work with transport staff physicians and other hospital personnel and patients required
