Job Description
The Senior Case Manager at The Jewish Board plays a pivotal role in supporting adults with serious mental illness in achieving independence. This position involves managing complex cases, advocating for residents, and ensuring their needs are met through personalized recovery goals.
Key Responsibilities
Manage a caseload and collaborate with residents on service planning
Evaluate and assess residents' independent living skills
Advocate for residents with service providers
Maintain documentation of resident interactions
Participate in intake, assessments, and discharge conferences
Required Qualifications
Bachelor of Arts (BA) or Bachelor of Science (BS) degree
Experience working with individuals living with serious mental illness, substance use disorders, or trauma
In-depth understanding of mental illness and its community impact
Preferred Qualifications
At least 1-year experience in Human Services
Familiarity with Microsoft Office suite
Valid driver's license and willingness to drive agency van
