Job Description
Join Optum as a Nurse Practitioner or Physician Assistant to provide essential care for patients transitioning from rehab to home. This role allows you to leverage your expertise while working independently to support a vulnerable population during critical healthcare transitions.
Key Responsibilities
Perform comprehensive assessments and document findings
Ensure accurate ICD10 coding and documentation
Address and document quality elements
Conduct initial medication reviews
Complete mandatory educational and training requirements
Travel between care sites as required
Communicate medical concerns to patients and team members
Required Qualifications
Certified Nurse Practitioner through a national board
Active and unrestricted APC license in the State of Connecticut
Current active DEA licensure/prescriptive authority or ability to obtain post-hire
Access to reliable transportation for travel to patient sites
Availability to work 10 hours per week
Ability to gain a collaborative practice agreement, if applicable
Preferred Qualifications
1+ years of hands-on post grad experience within Long Term Care
Ability to lift a 30-pound bag and navigate stairs
Understanding of Geriatrics and Chronic Illness
Understanding of Advanced Illness and end of life discussions
Proficient computer skills including electronic medical records
Ability to develop and maintain positive customer relationships
Adaptability to change
