Key Responsibilities
Identify ongoing needs and services of residents
Ensure proper follow through and documentation for residents' health care services
Assist Resident Care Director in resident evaluations
Conduct monthly wellness visits for all residents
Monitor residents who have experienced falls or incidents
Document all pertinent information in resident wellness files
Maintain communication with residents' families
Contact attending physicians as necessary
Ensure weights and vital signs are obtained monthly
Maintain wellness files according to policies and regulations
Provide clinical support to community team members
Understand and follow infection control practices
Regularly monitor residents' medication and treatment profiles
Participate in quality assurance and clinical meetings
Required Qualifications
Graduate of approved college/school of nursing
Current state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN)
Minimum of one year experience in home health, assisted living, hospital, or long-term care environment
Knowledge of nursing practices, techniques, and methods applied to health and wellness resident care
Assessment skills
Effective communication skills
Organizational and time management skills
Good judgment, problem-solving, and decision-making skills
Proficiency in computer skills, Microsoft Office, and Sunrise applications

Ability to handle multiple priorities
Ability to work weekends, evenings, and flexible hours
