Key Responsibilities
Assess, plan, implement, and coordinate care management activities
Develop personalized care/service plans for members
Coordinate access to services and resources for members
Monitor progress towards care plan goals
Provide psychosocial and resource support to members and caregivers
Collaborate with healthcare providers to ensure member needs are met
Collect and maintain compliance documentation for care management activities
Perform on-site visits to assess member needs
Educate care managers and members on healthcare options and benefits
Serve as a Subject Matter Expert (SME) for the care navigation team
Required Qualifications
Bachelor’s degree
4 – 6 years of related experience
Graduate from an Accredited School of Nursing (if holding clinical licensure)
Preferred Qualifications
Current state’s clinical license
CMS Florida licensed practical nurse with pediatric experience
Master’s degree in social work with professional pediatric care experience
