Job Description
Join The Jewish Board as a Case Associate and make a significant impact in the lives of clients by providing essential support and case management. Work closely with clients in their communities to develop action plans and connect them with vital resources, ensuring they lead safe and independent lives.
Key Responsibilities
Supply appropriate interventions to ensure client care
Develop action plans to identify and utilize resources for client care
Make routine community contacts
Locate and utilize appropriate community resources
Evaluate living conditions through home visits
Provide structure and guidance in independent living skills
Run groups under supervision of social worker
Evaluate clients for individualized treatment programs
Identify treatment or service providers and facilitate referrals
Perform individual/group counseling
Conduct home visits assessing family and child safety needs
Provide case management to clients
Perform community outreach and establish linkages
Required Qualifications
BA Degree
Experience in a community or social service group
At least five years of experience as a Case Assistant in a community or social service group
Preferred Qualifications
Bilingual Haitian-Creole
