Key Responsibilities
Link adults and children with chronic health conditions to necessary services
Develop individualized care plans
Provide care management services and track referrals
Conduct assessments of medical and behavioral health needs
Respond to member crises and provide outreach services
Required Qualifications
Bachelor's degree in social work, psychology, nursing, or related field
Two years of experience in direct services or case management for mentally disabled or chronically ill individuals
Experience working in interdisciplinary teams
Experience providing care management or care coordination in medical or behavioral health environments
Preferred Qualifications
Fluency in a second language (Spanish, Russian, or Creole)
Experience working with children or specific target populations
