Key Responsibilities
Develop, coordinate, and maintain therapeutic services and activities
Work alongside an interdisciplinary team to help patients maintain or improve their level of independence
Provide care in patients' homes
Required Qualifications
Bachelor's Degree from an accredited Physical Therapy Program
Master's Degree from an accredited Physical Therapy Program
D.P.T. from an accredited Physical Therapy Program
National Provider BLS - American Heart Association within 30 days of hire
Oregon Physical Therapist License upon hire
Washington Physical Therapist License upon hire if practicing in this state
1 year clinical experience in Home Health or healthcare setting or active enrollment in the Providence postgraduate preceptor program
