Key Responsibilities
Conduct Adult/Geriatric Assessments, medical history, diagnosis and treatment
Provide health education and referrals
Collaborate with physicians, case managers, and other medical personnel
Provide services in various venues including home visits and telehealth
Required Qualifications
Master’s Degree or equivalent experience
Current RN and NP licensure with prescription authority
Board certified by AANP, ANCC, or AACCN
Current BLS, ACLS, or CPR certification
Minimum of 1 year experience as a Nurse Practitioner
Strong computer skills and familiarity with health/medical record software
