Join MGA Homecare as a Transitional Care Team Nurse, where you'll provide essential nursing care to pediatric patients transitioning from hospital to home. You'll coordinate care and support families, ensuring a smooth discharge process across the Dallas metroplex.
Key Responsibilities
Provide nursing care and assist in transitioning patients from hospital to home
Assist with hospitals and agency to facilitate hospital discharge and prevent re-hospitalization
Assist with care coordination with patient care team
Provide education to patient and family caregivers as identified
Train new permanent nurses when identified
Meet new families at the hospital and help set expectations prior to discharge
Help assist with office New hire training classes
Help assist with office clinical tasks as needed
Provide PRN nursing coverage in the Dallas metroplex area
Required Qualifications
Current state license as a Registered Nurse
Minimum of two years of nursing experience in a Home Health or Acute setting
Current CPR card
Current health certificate
Current state driver’s license
Reliable transportation and automobile insurance
Attention to detail and time management skills
Effective problem-solving and conflict resolution
Good organization and effective communication
Preferred Qualifications
BSN
Trach and Vent experience
NICU/PICU experience
