Join The Jewish Board as a Case Planner, where you will work closely with families to provide essential child welfare services, ensuring safety and stability. Your role will focus on assessing family needs, providing counseling, and connecting families to vital community resources to enhance their well-being.
Key Responsibilities
Provide Child Welfare Preventive Services within the client's home and community
Assess and address safety and risk concerns related to children's health and well-being
Evaluate living conditions of clients through home base contacts
Provide supportive case work counseling
Identify and provide access to specific concrete services and community resources
Collaborate with external resources such as schools and mental health providers
Conduct specialized groups for children, adolescents, and parents
Complete weekly progress notes and timely Family Assessment Service Plan Reports
Attend required trainings and conferences
Carry a caseload size as identified by program service model
Monitor children regarding their medical needs
Perform outreach to communities and establish linkages with other groups
Required Qualifications
BA or Masters level degree in Psychology, Social Work, Counseling or a related subject
Basic social work knowledge and understanding of stages of change
Ability to provide in-home child welfare services
Ability to establish rapport and therapeutic relationship with families
Preferred Qualifications
Understanding of the use of family therapy principles to provide services
Understanding of early childhood development and related theories and models of practice
Strong assessment skills to evaluate dynamics and interactions within the family
Knowledge and experience with the Connections system
