Job Description
Join The Valley Health System as an RN Case Manager to ensure quality healthcare outcomes through personalized discharge planning and effective utilization management. Collaborate with patients and healthcare teams in a rewarding environment focused on delivering exceptional care.
Key Responsibilities
Establish safe, individualized discharge plans
Provide timely utilization management services
Collaborate with patients, families, and healthcare teams to achieve quality healthcare outcomes
Required Qualifications
Graduate of an accredited school of nursing
Current RN license in the State of Nevada
Minimum three years experience in varied clinical settings
Preferred Qualifications
Two years experience in Utilization Review, Utilization Management or Case Management
Knowledge of social and physical factors affecting functional status at discharge
Knowledge of community resources for post-discharge needs
