Key Responsibilities
Develops and facilitates complex care management activities for mental and behavioral health needs
Evaluates member needs via phone or in-home visits
Develops ongoing care plans for high acuity members
Coordinates care between members, families, and community resources
Monitors care plans and provides recommendations
Educates members and families on healthcare options
Documents and maintains member information for compliance
Required Qualifications
Master's degree in Behavioral Health or Social Work
Degree from an Accredited School of Nursing
2 - 4 years of related experience
Licensed Master's Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN
Preferred Qualifications
Bilingual candidates
Experience in pediatric care
