Job Description
Join Optum as an Associate Patient Care Coordinator and play a vital role in enhancing health outcomes through patient outreach and appointment scheduling. With a focus on comprehensive annual screenings, you'll connect patients with the care they need while benefiting from a culture of inclusion and career growth.
Key Responsibilities
Patient telephonic outreach to schedule health events for annual screenings
Collect and verify demographic information and insurance eligibility
Provide timely notifications and reminders for upcoming appointments
Follow up on rescheduled and cancelled appointments
Enter dispositions for all calls in WellMed system
Monitor call volume to ensure compliance with contact frequency
Utilize telephonic software for outreach and inbound calls
Maintain effective relationships with patients and staff
Perform related duties as assigned
Required Qualifications
High School Diploma/GED (or higher)
1+ years of experience with general office procedures
Advanced level of proficiency with communication and customer service skills both in person and via phone
Basic level of proficiency with Microsoft applications
Access to reliable transportation & valid US driver’s license
Preferred Qualifications
1+ years of related work experience including data entry
ICD 10 and CPT coding experience
Prior experience with EMR computer applications
Working knowledge of medical terminology
Bilingual in English & Spanish
