Compensation:

• Start-of-Care Visits: $150–$200 per visit (based on experience and case complexity)
• Follow-Up Visits & Discharges: Slightly lower rate due to reduced time requirements

Role Overview

Seeking a skilled Home Health Behavioral Health RN to provide psychiatric care and case management in a home health setting. The role involves conducting psychological assessments, implementing interventions, managing patient cases, and collaborating with families and healthcare professionals to ensure optimal patient outcomes.

Key Responsibilities

• Perform psychiatric evaluations and interventions in patients' homes
• Manage mental health cases, primarily within the geriatric population
• Coordinate care with multidisciplinary teams
• Maintain accurate documentation and communication with patients and families
• Travel within a 20–30 mile radius to visit patients

Qualifications

• Active RN license in California
• Home Health experience required
• Psychiatric/Mental Health experience required
• CPR certification
• Strong communication and interpersonal skills
• Ability to work independently and within a team

Additional Details

• Schedule: Monday–Friday; nurses schedule their own visits during daytime hours
• Patient Volume: Typically 5–7 patients per day
• Mileage Reimbursement: Provided for patient visits
• Sign-On Bonus: Available only for full-time hires who remain fully productive for six months; paid out over the following year
• Relocation Assistance: Not offered unless candidate commits to full-time, productive schedule and has prior home health experience

Start-of-Care Visits: $150–$200 per visit (based on experience and case complexity)
