Key Responsibilities

  • Provide restorative and rehabilitative services
  • Collaborate with primary care and interdisciplinary teams
  • Conduct initial assessments and reassessments of participants
  • Develop and implement individualized care plans
  • Instruct and supervise therapy program aides
  • Evaluate participants’ home environments
  • Maintain accurate documentation and confidentiality

Required Qualifications

  • Bachelor’s degree in Occupational Therapy
  • Graduate of an accredited Occupational Therapy program
  • Licensed Occupational Therapist in California
  • Current CPR and First Aid certification
  • Valid California driver’s license
  • Access to a reliable vehicle
  • Ability to travel to multiple work sites

Preferred Qualifications

  • One year of experience as a Licensed Occupational Therapist with the frail elderly population
