Key Responsibilities
Provide restorative and rehabilitative services
Collaborate with primary care and interdisciplinary teams
Conduct initial assessments and reassessments of participants
Develop and implement individualized care plans
Instruct and supervise therapy program aides
Evaluate participants’ home environments
Maintain accurate documentation and confidentiality
Required Qualifications
Bachelor’s degree in Occupational Therapy
Graduate of an accredited Occupational Therapy program
Licensed Occupational Therapist in California
Current CPR and First Aid certification
Valid California driver’s license
Access to a reliable vehicle
Ability to travel to multiple work sites
Preferred Qualifications
One year of experience as a Licensed Occupational Therapist with the frail elderly population
