Home Health – Physical Therapist (Days)

Position Overview

We are seeking an experienced Physical Therapist (PT) with home health experience to join Muir Home Health on a 13-week contract. This is a full-time opportunity, Monday–Friday, providing patient-centered care within the home setting for a primarily geriatric population.

Assignment Details

• Contract Length: 13 weeks
• Schedule: Monday–Friday, 8:00 am – 5:30 pm (visits typically begin 9–10 am)
• Hours: 40 per week
• Shift: Days
• On-Call: None
• Extension Opportunities: Yes
• RTO: Accepted

Responsibilities

• Provide skilled physical therapy in the home health setting
• Manage a caseload of ~6 patients per day (assigned the night before; PT calls patients to schedule)
• Deliver care tailored to the needs of geriatric patients
• Collaborate with a multidisciplinary team, including RNs and OTs
• Document care in the EMR (Oasis/HCHB – training provided if needed)

Requirements

• Active California PT License
• Current BLS Certification
• Minimum 1 year of home health experience
• Familiarity with HCHB or Oasis preferred (training available if not experienced)
• Strong organizational and communication skills

Additional Information

• Orientation: 1 week (HR, EMR training, shadow/mentor sessions before independent visits)
