Job Description

Join Sunshine State Health Plan as a Case Manager, where you will assess and coordinate holistic care for our members, ensuring quality and cost-effective healthcare outcomes. This role involves collaborating with healthcare providers and educating members and families on available services.

Key Responsibilities

  • Assess, plan, implement, and coordinate holistic care management activities.
  • Develop personalized care/service plans for long-term care members.
  • Educate members and their families/caregivers on available services and benefits.
  • Monitor and adjust care plans based on member needs.
  • Identify problems/barriers in long-term care management.
  • Provide resource support for various member needs.
  • Collaborate with healthcare providers to facilitate member care.
  • Perform home visits to assess member needs.
  • Maintain compliance with state and federal regulations.

Required Qualifications

  • Bachelor's degree
  • 4 – 6 years of related experience
  • Licensed Practical Nurse
  • 2 years of Pediatric experience
