Job Description
Join The Jewish Board as a Case Associate to make a significant impact in the lives of clients by developing action plans, providing support, and utilizing community resources. This role is vital in ensuring client care and independence through individual and group counseling.
Key Responsibilities
Function as a case planner and ensure appropriate interventions for client care
Develop action plans and utilize existing resources for clients
Conduct routine community contacts and home visits
Evaluate living conditions and provide individualized support
Run groups under supervision of a social worker
Perform individual/group counseling
Provide case management and access to services
Conduct community outreach and establish linkages
Required Qualifications
BA Degree
Experience in a community or social service group
At least five years of experience as a Case Assistant
Preferred Qualifications
Bilingual Haitian-Creole
