Job Description

Key Responsibilities
Evaluate patient needs and develop individualized care plans
Coordinate interdisciplinary services and educate patients, families, and caregivers
Monitor patient progress to achieve therapeutic goals
Manage documentation and compliance with care standards
Participate in performance improvement initiatives
Required Qualifications
Master's in Speech Therapy
1 year relevant clinical Speech therapy experience
Speech Therapist Certified in State of Practice
Valid state Driver's License with current car insurance
Preferred Qualifications
Recent experience in a Medicare-certified home health agency as a case-manager
Certificate for OASIS Specialist Clinical
Home Health Case-Manager Certification
