Job Description
Join The Jewish Board as a Case Associate and make a significant impact in the lives of New Yorkers. You'll provide essential case management and support, helping clients navigate their care through community resources and personalized treatment plans.
Key Responsibilities
Function as a case planner and supply appropriate interventions for client care
Develop action plans and utilize existing resources for client care
Conduct routine community contacts and home visits
Evaluate living conditions and assess client needs
Provide structure and guidance to clients in independent living skills
Run groups under supervision of social worker
Evaluate clients for individualized treatment programs
Perform individual/group counseling as needed
Identify and facilitate client referrals to service providers
Perform community outreach and establish linkages with other organizations
Maintain necessary documentation and progress notes
Required Qualifications
BA Degree with experience in a community or social service group
At least five years of experience as a Case Assistant in a community or social service group
Preferred Qualifications
Bilingual Haitian-Creole
