Job Description
Join the Youth Assertive Community Treatment (ACT) Team as a Licensed Behavioral Health Clinician, providing essential support to children and families facing significant behavioral health challenges. Collaborate with a multi-disciplinary team to deliver tailored treatment, promote family engagement, and facilitate successful transitions back into the community.
Key Responsibilities
Provide treatment to the child and their family/caregivers to address clinical needs
Engage child/youth and family in developing, reviewing, and modifying a course of care
Assess risk, develop safety plans, and provide crisis intervention services
Develop and implement person-centered care plans
Actively participate as part of a multi-disciplinary team
Monitor appropriate discharge and ongoing care planning
Establish collaborative working relationships with community providers
Complete casework documentation and collect reports
Provide on-call after hour crisis intervention services
Required Qualifications
Licensure (LMSW, LCSW, LMFT, LCAT) in a Behavioral Health or related field
A minimum of one (1) year of experience in providing direct services to children with serious emotional disturbance and their families
