Join Professional Case Management as a Senior Case Manager RN, where you can create your own schedule and enjoy great work/life balance. You will lead a team in providing critical in-home care to former Nuclear Weapons Workers while ensuring compliance with agency policies and state regulations.
Key Responsibilities
Provide direct care and case management of assigned clients
Assist and collaborate with the regional director and other personnel to identify and improve services
Plan, implement, and evaluate care provided
Manage client care conferences as needed
Serve as a local on-site clinical resource
Review and evaluate clinical documentation for accuracy and completeness
Participate in personnel training and education
Assist with marketing activities as requested
Required Qualifications
Graduate of a state approved school of professional registered nursing
Current, unrestricted RN license in the state(s) of practice
Minimum of two (2) years nursing experience including one (1) year in home care or closely related field
Current CPR certification
Preferred Qualifications
BSN preferred
One (1) year of supervisory and/or case management experience preferred
