Job Description
Join The Jewish Board as a Case Associate and make a significant impact in the lives of New Yorkers. You will develop action plans and provide essential client care while utilizing community resources and delivering support through counseling and case management.
Key Responsibilities
Develop action plans for client care
Conduct routine community contacts
Evaluate living conditions through home visits
Provide structure and guidance in independent living skills
Run groups under supervision of the social worker
Evaluate clients for individualized treatment programs
Facilitate referrals to treatment or service providers
Perform individual/group counseling
Conduct community outreach
Provide case management
Document necessary notes and progress assessments
Required Qualifications
BA Degree
Experience in a community or social service group
At least five years of experience as a Case Assistant in a community or social service group
Preferred Qualifications
Bilingual Spanish
Haitian-Creole
