Job Description

Join Humana as a Field Care Manager in Behavioral Health, where you'll deliver integrated care through face-to-face and telephonic assessments. You will play a vital role in enhancing health outcomes for Virginia’s Medicaid population by coordinating services and providing personalized support.

Key Responsibilities

  • Conduct face-to-face and telephonic assessments with members
  • Provide integrated care and personalized support to members
  • Engage members in their communities to build trust
  • Complete required assessments including Comprehensive Risk Assessment
  • Coordinate behavioral health and medical services
  • Improve member’s health literacy and address social needs
  • Serve as the coordinator for the member’s interdisciplinary care team
  • Facilitate interdisciplinary care team meetings
  • Conduct biannual and quarterly face-to-face visits
  • Collaborate with internal departments and community organizations
  • Ensure compliance with regulatory requirements

Required Qualifications

  • Bachelor’s degree in social work, psychology or related field
  • Virginia licensed LMHP, LPC or LCSW (No supervisees or provisional licenses)
  • Minimum of 2 years of post-degree clinical experience in behavioral health setting
  • Case management experience with complex SMI or SED population
  • Must reside in Virginia, specifically Northern/Winchester or Western/Charlottesville
  • Ability to travel to region-based facilities and homes for assessments
  • Exceptional communication and interpersonal skills
  • Ability to work with minimal supervision
  • Proficiency in electronic medical records and Microsoft Office
  • Valid driver's license, car insurance, and reliable transportation

Preferred Qualifications

  • Case Management Certification (CCM)
  • 3-5 years of in-home assessment and care coordination experience
  • Experience with Medicare, Medicaid and dual-eligible populations
  • Experience with high risk pregnant and postpartum population with BH needs
  • Field case management experience
  • Knowledge of community health and social service agencies
  • Previous managed care experience
  • Bilingual preferred (Spanish, Arabic, Vietnamese or other)
