As a Case Associate at The Jewish Board, you'll empower adults with serious mental illness to achieve independence in their community. Utilizing a compassionate, person-centered approach, you will support residents in their recovery journey while collaborating with various service providers.
Key Responsibilities
Support residents in meeting individual goals and independent living skills
Advocate for residents and link them to community-based services
Conduct monthly inspections of units/apartments
Complete timely documentation of resident interactions
Participate in staff meetings and training
Required Qualifications
AA degree and two years of experience in a community mental health setting
HS/HSE degree and at least five years of experience in a community mental health setting
Preferred Qualifications
Familiarity with Microsoft Office suite
Valid driver's license and willingness to drive agency van
