Job Description

Join Cooper University Health Care as a Population Care Coordinator where you'll play a vital role in enhancing patient care through coordination and team-based approaches. You'll assess and implement healthcare options while collaborating with patients and their providers, ensuring improved health outcomes for diverse populations.

Key Responsibilities

  • Support Patient-centered Medical Home (PCMH) care coordination
  • Assess, plan, implement, coordinate, monitor, and evaluate healthcare options
  • Collaborate with health and community providers for patient goal-setting
  • Provide outreach, care management, and education for disease self-management
  • Monitor and evaluate community resources for individual health needs
  • Conduct assessments and develop nursing treatment plans
  • Assist in developing evidence-based practices and care plans
  • Collect data on quality metrics and participate in improvement initiatives

Required Qualifications

  • 5+ years of clinical experience
  • Current NJ RN License

Preferred Qualifications

  • Experience in primary care, population or case management
  • BSN (Bachelor of Science in Nursing)
