Key Responsibilities
Support Patient-centered Medical Home (PCMH) care coordination
Assess, plan, implement, coordinate, monitor, and evaluate healthcare options
Collaborate with health and community providers for patient goal-setting
Provide outreach, care management, and education for disease self-management
Monitor and evaluate community resources for individual health needs
Conduct assessments and develop nursing treatment plans
Assist in developing evidence-based practices and care plans
Collect data on quality metrics and participate in improvement initiatives
Required Qualifications
5+ years of clinical experience
Current NJ RN License
Preferred Qualifications
Experience in primary care, population or case management
BSN (Bachelor of Science in Nursing)
