Job Description
Join Optum as a Licensed Vocational Nurse, providing essential home health care to patients under the supervision of a registered nurse. This role emphasizes patient education and collaboration, making a significant impact on community health.
Key Responsibilities
Provide direct patient care as defined in the state Nurse Practice Act
Implement care plans initiated by the registered nurse
Document care accurately and timely
Educate patients and families about diagnosis and care plans
Coordinate home health services with the clinical team
Follow infection control and safety guidelines
Participate in personal and professional growth
Communicate with physicians regarding patient needs and conditions
Required Qualifications
High school diploma or equivalent
Graduate of an accredited practical nurse or vocational nursing program
Current unrestricted license in Texas as a practical nurse or licensed vocational nurse
Current CPR-BLS certification or ability to obtain prior to start
2+ years of clinical experience in community home health or medical/surgical setting
Thorough knowledge of current nursing practice and standards
Ability to comply with professional standards and practices
Valid Texas driver's license and maintain personal auto insurance
Preferred Qualifications
Experience with electronic medical record applications
Ability to prioritize and communicate objectives clearly
Ability to interact with multidisciplinary teams
Excellent observation, verbal and written communication skills
Evidence of independent nursing practice
Basic computer skills including Microsoft Word and Outlook
Excellent organizational and prioritization skills
