Job Description

Key Responsibilities
Identify ongoing needs and services of residents.
Ensure proper follow-through and documentation for residents with changes in healthcare services.
Assist Resident Care Director in conducting evaluations.
Conduct monthly wellness visits for residents.
Monitor and follow up on residents who have experienced incidents.
Document all pertinent information in resident wellness files.
Maintain communication with residents' families regarding medical needs.
Contact attending physicians when necessary.
Ensure weights and vital signs are obtained monthly.
Maintain wellness files according to policies and regulations.
Provide clinical support to community team members.
Follow infection control practices.
Regularly monitor medication and treatment profiles.
Participate in quality assurance meetings.
Required Qualifications
Graduate of approved college / school of nursing.
Current state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN).
Minimum of one (1) year experience in home health, assisted living, hospital, or long-term care environment.
Knowledge of nursing practices, techniques, and methods applied to health and wellness resident care.
Good assessment skills.
Ability to handle multiple priorities.
Effective written and verbal communication skills.
Organizational and time management skills.
Good judgment, problem-solving, and decision-making skills.
Proficiency in computer skills, Microsoft Office, and Sunrise applications.
