Key Responsibilities: Assess patient needs and develop individualized care plans Administer medications and perform clinical procedures as prescribed Monitor and document patient conditions, interventions, and outcomes Provide education and emotional support to patients and families Collaborate with interdisciplinary care teams to ensure quality and continuity of care Deliver compassionate hospice care focused on comfort, dignity, and quality of life Requirements: Current and active Montana RN license – Required BLS certification – Required Experience in both Home Health and Hospice – Required Valid driver’s license and clean driving record – Required Strong assessment, documentation, and communication skills Ability to work independently in patients’ homes
