Key Responsibilities
Conduct comprehensive in-home health risk assessments
Take patient history and vital signs
Provide written documentation of patient visits
Place case management referrals
Communicate with patients, caregivers, and other providers
Observe OSHA Universal Precautions
Maintain patient confidentiality
Participate in required meetings and professional development activities
Required Qualifications
Active/unrestricted medical license
Active CPR Certification
Board eligible or board certified in family medicine
Outstanding EHR skills
Preferred Qualifications
Geriatric training/experience
Skill in teamwork and maintaining effective working relationships with patients, medical staff, and the public
