As a Referral Coordinator, you will assist older adults by providing information on community resources and making referrals to the services best suited to their needs. You will work in a supportive environment, ensuring compassionate and efficient assistance while maintaining accurate records and following up on referrals.
Key Responsibilities
Receive incoming calls to identify needs and make appropriate referrals
Record details of each call and maintain client information
Research and retrieve information on public and private resources
Connect callers with suitable agencies and follow up on assistance
Offer unbiased information about long-term care or support services
Uphold confidentiality and privacy standards
Maintain case narratives and note important details
Participate in outreach activities to increase public awareness
Assist with emergency response efforts as assigned
Support the team in gathering and organizing information
Required Qualifications
Bachelor’s degree in a human services field
OR Associate’s degree in a human services field with at least two years of relevant experience
OR High school diploma (or GED) plus four years of directly related experience
Background in information/referral services, case management, call center support, social services, or similar role
Strong verbal communication skills
Ability to work independently and collaboratively
Proficiency in Microsoft Office and familiarity with databases and internet research
Accurate typing at a minimum of 35 words per minute
Knowledge of or willingness to learn about aging services and community resources
