Physical Therapist – LTC/SNF - Travel Position

Job Description:

We are seeking a compassionate Physical Therapist to join a skilled nursing and long-term care facility in Franconia, NH, on a travel contract. The Physical Therapist will assess residents, develop individualized treatment plans, and deliver therapy services designed to improve mobility, restore function, and promote independence. The ideal candidate is patient-centered, detail-oriented, and experienced in geriatric rehabilitation.

Key Responsibilities:

Conduct patient evaluations to determine mobility, functional status, and therapy needs.
Develop and implement individualized treatment plans in coordination with physicians and care teams.
Provide therapeutic interventions such as gait training, strengthening, balance activities, and mobility support.
Educate residents, families, and caregivers on therapy goals, safety, and the use of assistive devices.
Document evaluations, progress notes, and treatment outcomes accurately in compliance with facility and regulatory standards.
Collaborate with interdisciplinary staff to ensure coordinated, patient-centered care.
Promote safety, independence, and improved quality of life through evidence-based therapy practices.

Qualifications:

Doctor of Physical Therapy (DPT) or Master’s in Physical Therapy from an accredited program.
Current and valid Physical Therapist license in New Hampshire (or compact license).
Minimum of 1 year of recent PT experience, preferably in skilled nursing or long-term care facilities.
Current certification in Basic Life Support.
COVID-19 vaccine and recent tuberculosis test.
Strong clinical, communication, and organizational skills.
Compassionate, patient-centered approach with experience supporting geriatric populations.
