Job Description
Join BoldAge PACE as a PCP Nurse Practitioner and make a meaningful impact on the lives of seniors. Provide comprehensive care in an all-inclusive program, working collaboratively with healthcare professionals to enhance the quality of life for older adults.
Key Responsibilities
Provide direct and indirect care to participants in the PACE Health Clinic and Day Center
Perform health histories, physical exams, and participant teaching
Order, interpret, and follow up on lab tests
Manage complex clinical situations
Coordinate participant care under supervision of attending physician
Participate as a member of the Interdisciplinary Team
Document participant changes in the medical record
Encourage participants to have Advanced Directives
Required Qualifications
Master’s degree in clinical nursing from accredited Nurse practitioner program
RN licensure in perspective state
Family certification from Nurse Practitioner program
1 year of experience working with a frail or elderly population preferred
Reliable transportation and valid driver's license
Medically cleared for communicable diseases and up-to-date immunizations
Pass a comprehensive criminal background check
Preferred Qualifications
3-5 years nursing experience with Nurse Practitioner experience preferred
