Key Responsibilities
Administer evaluations and develop individualized treatment strategies
Provide targeted case management and supportive counseling
Facilitate linkage and referral to services and resources
Advocate for clients to ensure continuity of services
Conduct regular follow-ups with clients
Ensure crisis response planning procedures are in place
Coordinate warm hand-offs with other staff
Participate in treatment and discharge planning
Required Qualifications
Bachelor’s Degree in a Human Service field
Valid driver’s license and vehicle
Demonstrated knowledge of basic computer skills
Preferred Qualifications
West Virginia Social Work License
Experience in behavioral health and/or substance use disorders
