Requirements:

1. OASIS + HCHB EXPERIENCE REQUIRED
2. Minimum 2 years experience as Home Health RN
3. Current RN licensure in state of practice
4. Current CPR certification required
5. Current Drivers License, vehicle insurance, and access to a dependable vehicle or public transportation
6. COVID - must be fully vaccinated at time of submission
