Contract Length: 13–24 weeks
Job Highlights

Patient Load: ~5 visits/day
Charting: Epic
Scrub Color: Royal Blue
Mileage: Reimbursed at $0.55/mile
On-Call: 1 weekend/month + 5–6 on-call shifts/month
RTO: Max 7 days
Guaranteed Hours: 3 shifts per 13 weeks (must be full-length shifts)
Local Rule: Must live 75+ miles from facility
Experience: 1–2 years Home Health Hospice (first-time travelers welcome!)
Responsibilities:
Provide skilled nursing services, preventive and rehabilitative procedures, and patient education for self-care.
Act as case manager to coordinate all aspects of patient care.
Develop, implement, and document individualized care plans in collaboration with the interprofessional team.
Assess patient needs, preferences, and barriers to care (emotional, spiritual, cultural).
Evaluate patient progress, update care plans in Epic EMR, and communicate with caregivers and healthcare team.
Ensure quality outcomes through evidence-based practice, safety, and continuous monitoring.
Requirements
Active IL RN License (no pending accepted)
BLS (AHA)
1–2 years of Home Health Hospice experience
Reliable transportation for mileage-based travel
